What do I need to do in the event of death or disability?

To apply for death and disability forgiveness, please follow these steps:

1.  Visit our secure Document Upload Center by clicking this link: Upload - (Document Upload Center) | College Ave (collegeavestudentloans.com).
2.  Select "Death or disability verification for loan forgiveness" in the dropdown menu.
3.  Proceed by filling out all fields and select which document you will be uploading.
4.  Upload the document, then click submit.

Acceptable documentation for each scenario:

• In the event of death:
  • Copy of Death Certification
• In the event of disability:
  • Social Security Statement (Benefit Verification Letter
    • Make sure this includes the start date of the benefit.
  • Medical Documentation
    • Documentation that supports disability.
  • Reduction of income documentation
    • Any additional documents that verify reduction of income.
